If you've been trying to conceive and things aren't working the way you expected, it's natural to start looking for answers. Ovulation disorders, conditions that disrupt or prevent the monthly release of an egg from the ovary, are one of the most common reasons women find it difficult to get pregnant. The most complete form is anovulation, where no egg is released at all. A milder form, oligovulation, is when ovulation happens but infrequently or unpredictably. There are several types of ovulation disorders. The most common include PCOS, thyroid disorders, and elevated prolactin levels, among others. Symptoms can range from irregular periods and difficulty conceiving to hormonal changes like acne or hair loss. Most ovulation problems are diagnosable and treatable. With the right support, many women with ovulation disorders go on to conceive, whether through lifestyle changes, medication, or assisted reproductive techniques.
Ovulation disorders are conditions that disrupt the body's normal process of releasing an egg. In a typical menstrual cycle, one egg matures and is released from an ovary roughly every 28–35 days. When this doesn't happen consistently or doesn't happen at all, it's classified as an ovulation disorder.
The most severe form is anovulation, when no egg is released. A milder form is oligovulation, where egg release happens but on an irregular or infrequent basis. Both reduce the chances of natural conception.
Several underlying conditions can cause ovulation problems. Here are the most common ones.
Polycystic Ovary Syndrome (Now referred to as PMOS) is the most common cause of ovulation disorders. According to a 2021 review published in Frontiers in Endocrinology, it affects between 8–13% of women of reproductive age worldwide.
It occurs when the ovaries produce excess androgens (male hormones that women also produce in small amounts), which interfere with egg development and release. Many women with PCOS have multiple small follicles (fluid-filled sacs in the ovary) that don't mature fully.
The hypothalamus is an area in the brain responsible for regulating the hormones that signal ovulation. When the hypothalamus fails to function normally due to factors like stress, extreme weight loss, or rigorous physical exercise, it may inhibit the release of hormones that would otherwise cause ovulation. This is also known as hypothalamic amenorrhoea.
Prolactin is a hormone that is secreted from the pituitary gland (a tiny gland in the brain). Prolactin is normally associated with breastfeeding. In cases where there is an elevated level of prolactin other than breastfeeding (hyperprolactinaemia), it will affect the process of ovulation. This happens due to medications, a benign tumour in the pituitary gland, among other factors.
The thyroid gland is responsible for regulating various functions in the body, such as reproductive hormones. Hypothyroidism (underactive thyroid) and hyperthyroidism (overactive thyroid) both affect the ovulation process. Thyroid disorders are common in India. It was found that, in 2011, the prevalence of thyroid disorders in India was around 42 million, as stated in a research article in the Indian Journal of Endocrinology and Metabolism.
POI is characterised by dysfunction of the ovaries before reaching the age of 40. As a result, there is a decrease in oestrogen production and in the number of eggs, which makes ovulation quite uncommon. According to the National Institutes of Health (NIH), POI affects approximately 1 in 100 women under the age of 40.
Symptoms vary depending on the cause. Here are the most common signs to be aware of.
Irregular periods are often the first sign of an ovulation problem. If your cycle is consistently shorter than 21 days, longer than 35 days, or simply unpredictable from month to month, it may indicate that ovulation is not occurring regularly.
If you have been trying to conceive for 12 months, or 6 months if you are over 35, without success, an ovulation disorder could be a contributing factor.
Conditions like PCOS can cause acne, excess facial or body hair (called hirsutism) or hair thinning on the scalp. These are signs of elevated androgen levels affecting the body.

The process of menstruation requires an exact timing of hormonal secretion. Follicle-stimulating hormone (FSH) plays a role in the development of eggs, and luteinising hormone (LH) plays a role in the release of eggs. Estrogen and progesterone are involved in the regulation of the uterine lining. An imbalance in any of these can interfere with ovulation.
Being significantly underweight or overweight can both disrupt hormone production. Body fat plays a role in oestrogen levels, so changes at either extreme of the weight spectrum can alter the cycle.
Chronic stress and intense physical training suppress hormonal signals from the hypothalamus. This is one of the common causes of irregular ovulation, particularly in athletes or those going through significant emotional strain.
PCOS, thyroid disorders, uncontrolled diabetes, and high prolactin levels are among the most frequent medical causes of irregular ovulation. Treating the underlying condition often helps restore regular ovulation.
As women move through their late 30s and into their 40s, the number and quality of eggs in the ovaries naturally decline. This reduced ovarian reserve, measured by a hormone called AMH (anti-Müllerian hormone), can make ovulation less predictable over time.
Ovulation disorders and infertility are closely connected. The formation of an embryo presupposes the release of a normal egg cell on time during each menstrual cycle. If there are ovulation problems, then the chances of pregnancy are lower. But most women with ovulation problems manage to conceive.

Ovulation induction treatment uses medications to stimulate the ovaries to develop and release an egg. Common options include:
If thyroid dysfunction, hyperprolactinaemia, or some other identifiable disorder is the cause, then the treatment for this condition may induce ovulation on its own.
If ovulation induction treatment alone doesn't result in pregnancy, assisted reproductive techniques may be recommended. Intrauterine insemination (IUI) involves placing prepared sperm directly into the uterus around the time of ovulation. In vitro fertilisation (IVF) involves fertilising an egg in a laboratory and then transferring the resulting embryo to the uterus.
Not all ovulation disorders can be prevented. Conditions like POI and certain hormonal disorders are not within your control. That said, maintaining a balanced diet, managing chronic stress, avoiding extreme exercise regimens, keeping thyroid and blood sugar levels in control, and attending regular health screenings can support hormonal health and help identify problems early.
