In vitro fertilization (IVF) is a series of procedures that is used to help infertile couples get pregnant with the help of this fertility treatment. It also helps in preventing genetic problems to be passed on to the progeny and hence, assists with the conception of a child when the couple is unable to do it naturally.

During this treatment, the egg retrieval process is carried out in which mature eggs are collected from the woman’s ovaries and then fertilized by sperm in a lab. The fertilized egg known as embryo or embryos is then transferred into the woman’s uterus. A full cycle of IVF takes about three weeks of time. The process of IVF may differ slightly from one fertility clinic to another, but a typical IVF treatment follows these series of steps.

First, let’s talk about women-

Step 1: Suppressing the natural monthly cycle
The woman undergoing the treatment of IVF will be given medications that will suppress her natural monthly cycle. These medicines will be of very good help in the next stage of IVF treatment and will make it more effective.
The medication which is given to the woman is either in the form of a daily injection that she'll be trained to give herself, or in the form of a nasal spray. Now comes Step-2: Boosting the egg supply
Once the natural menstrual cycle is suppressed completely, the woman will be given a fertility hormone known as follicle stimulating hormone (FSH). This is another daily injection the woman will give herself after being trained about the same, usually for a period of about 10 to 12 days.
This hormone increases the number of eggs a woman’s ovaries produce, which means more number of eggs can be collected and then fertilized. Step-5: Fertilizing the eggs
The eggs which were retrieved in the earlier process are then mixed with the partner's sperm or the donor's sperm in a laboratory. After the duration of nearly 16 to 20 hours, they are observed to see if any of them have been fertilized.
The fertilized eggs or embryos continue to grow in the laboratory for up to 6 days, before they are transferred into the womb. Out of all the fertilized eggs, the best 1 or 2 embryos are chosen and then transferred.
After the egg retrieval process, the woman undergoing IVF treatment will be given hormonal medicines to help prepare the lining of the womb so as to receive the embryo. This is normally given either via a pessary or through an injection, or gel.

The Final Step: Embryo transfer

This procedure is simple as compared to egg retrieval and somewhat similar to having a cervical screening test, so it can be carried out without sedation. Age is a matter of concern here-

  • Women under the age of 37 should only go with a single embryo transfer in their first IVF cycle. In their second IVF cycle, they should again go with a single embryo transfer if one or more than one top-quality embryos are available. Doctors usually consider using 2 embryos if no good quality embryos are available to transfer. In the third IVF cycle, not more than 2 embryos should be considered for transfer.

  • Women between 37-39 years of age should opt for a single embryo transfer if there are 1 or more than 1 top-quality embryos while going through both first and second full IVF cycles and the transfer of double embryos should only be considered in the case if there are no top-quality embryos. In the third IVF cycle, not more than 2 embryos should be considered for transfer.

  • Women between 40-42 years of age may go through a double embryo transfer. If any suitable embryos are left unused then they may be kept frozen for future IVF attempts.

While the female partner’s eggs are being retrieved, the male partner will be asked to produce a fresh semen sample. The sperms collected are then washed and spun at a high speed so that the healthiest and the best sperm can be selected.

Finding out if you're pregnant
